Optional) How to take your basal body temperature  This form is somewhat optional, but I encourage you to try to get a few measurements, especially if your current health concerns include fatigue, lack of energy, cold hands and feet, depression, or high cholesterol. All women should make a special effort to obtain a few basal temperature measurements, as this information provides great insight into many women's hormonal concerns.  Again, you can also have your previous medical records sent to our office by completing this records request form and mailing it to your doctor's office.

 

The more information I have, the better I can understand your physiology and health status.  The better I understand you, the higher is our probability of helping you reach your health goals.
